Client Statement

I understand that:

1. Physical distancing of 2m. may not be possible at all points whilst in the salon

2. I am required to wear a covering over my nose and mouth in the salon, unless I have an exemption

3. I must sanitise my hands before or upon entering the salon

4. I must make all attempts to cover my mouth and nose in the event of coughing and/or sneezing and then immediately sanitise my hands

5. I will minimise the touching of common surface/areas

6. I understand the staff of Fusion-Southport will do everything possible to minimise the spread of COVID-19, but will not hold them responsible should I contract COVID-19.

I confirm that:

1. I am not currently positive for COVID-19 or waiting for the results of a test for COVID-19

2. I have not currently been asked to self-isolate by any government agency

3. I am not presenting with any of the following symptoms of COVID-19:

• High temperature, fever, chills or body aches (flu-like symptoms)

• Recent onset of a new continuous cough or Sore Throat

• Shortness of breath / Difficulty breathing

• Loss of normal sense of smell or taste

I will immediately notify the salon if I contract the virus within two weeks following my visit.
